The Housing Connection has been supporting people with intellectual and other disabilities within the North Shore area since 1981.

A support co-ordinator plays a critical role in supporting participants to navigate the NDIS system, understand their NDIS plans, connect into the broader system of services and to build their capacity to direct their supports independently over time. In this role you will be assisting NDIA participants and their families to connect to and coordinate informal, mainstream and funded supports in a complex service delivery environment and to report on participant goals and outcomes. It may also include resolving points of crisis, developing capacity and resilience in a participant’s network and coordinating supports from a range of sources.
This role would be based in Chatswood. We offer flexible working hours and access to fringe benefits.
We are seeking a full-time support coordinator that can work in the office.
